About the role

Key responsibilities & impact
  • Collaborate with clinic team members to provide evidence-based, patient-centered care
  • Help patients stay up to date on care plans and related health-improvement goals
  • Coordinate care for high-risk medically complex patients
  • Support patients on specific disease registries
  • Coordinate care for patients with chronic physical or intellectual disabilities
  • Facilitate care transitions, including pediatric-to-adult care, recent inpatient admissions and discharges, emergency department follow-up, and care facility discharges
  • Act as a liaison with patients’ family members and other agencies
  • Improve communication to support comprehensive whole-person care
  • Stay current on standards
  • Assist with clinical programs and processes

Requirements

What you’ll need
  • Associate Degree in Nursing from a CCNE- or ACEN-accredited nursing program
  • Current Ohio Registered Nurse license
  • CPR/BLS certification
  • Knowledge of triage procedures
  • Clinic, physician office, and/or urgent care experience
  • Ability to maintain quality, safety, and infection control standards
  • Knowledge of procedures and standards for medication and patient-care-aid administration
  • Ability to educate and clearly communicate disease information and care instructions to patients and families
  • Ability to convey information in writing and verbally
  • High interpersonal skills for handling sensitive and confidential situations
  • Demonstrated poise, tact, and diplomacy
  • Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions
  • Availability aligned with clinic operating hours: Monday–Friday, 8:00am–9:30pm; Saturday and Sunday, 8:00am–8:00pm
