About the role

Key responsibilities & impact

Collaborate with clinic team members to provide evidence-based, patient-centered care
Help patients stay up to date on care plans and related health-improvement goals
Focus on high-risk medically complex patients
Support patients on specific disease registries
Coordinate care for patients with disorders or chronic physical or intellectual disabilities
Support patients during care transitions, including pediatric-to-adult care, recent inpatient admissions and discharges, emergency-department follow-up, and care-facility discharges
Act as a liaison with patients’ family members and other agencies
Facilitate improved communication to provide comprehensive whole-person care
Stay current on standards
Assist with programs and processes

Requirements

What you’ll need

Associate Degree in Nursing from a CCNE- or ACEN-accredited nursing program
Current Ohio Registered Nurse license
CPR/BLS certification
Knowledge of triage procedures
Clinic, physician office, and/or urgent care experience
Ability to maintain quality, safety, and/or infection control standards
Knowledge of procedures and standards for administering medications and patient care aids
Ability to educate and clearly communicate with patients and families about disease, care, and treatment
Ability to convey information in writing and verbally
High interpersonal skills for sensitive and confidential situations
Poise, tact, and diplomacy
Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions
