About the role

Key responsibilities & impact
  • Monitors and evaluates drug therapy for appropriateness based on the patient’s health status, institutional criteria/guidelines, and cost efficient measures
  • Assumes responsibility for the management of pharmacy services of assigned areas for the goal of improving patient outcomes
  • Provides drug information to health care personnel and participates with appropriate medical team on patient care rounds
  • Precepts pharmacy students and residents and provides in-services/education to pharmacy and other health care providers when appropriate
  • Handles refill requests for Primary Care
  • Expedites the refill process, and saves providers' time responding to patients' refill requests, while promoting patient safety
  • Completes E-Consults for topics related to drug information, dose adjustments, order navigation, etc.

Requirements

What you’ll need
  • Completion of a PGY-1 Pharmacy Practice Residency (3 years related specialty experience can be accepted in lieu of PGY-1 Residency)
  • Doctor of Pharmacy (PharmD) preferred
  • Ambulatory experience preferred
  • License to practice Pharmacy by the LA Board of Pharmacy
  • Computer skills and dexterity for data entry and retrieval of patient information
  • Proficient with Windows-style applications and keyboard
  • Effective verbal and written communication skills
  • Strong leadership/teaching skills
