About the role

Key responsibilities & impact

Provide direct coaching support to clinical teams, both in-person and remotely
Assist care teams in obtaining and using data to create clinical/operational action plans
Support health center team adoption of technology solutions to drive practice transformation
Manage multiple concurrent clinical improvement projects
Identify, document and share best practice insights and actionable recommendations

Requirements

What you’ll need

Bachelor's degree in healthcare or related field, master's degree strongly preferred
Minimum of three (3) years of experience supporting health care teams in similar or relevant role strongly preferred
Strong process understanding of project lifecycle, highly independent and self-motivated
Demonstrated experience supporting individual clinicians, care teams, and clinic leaders with implementing practice changes in a clinical setting
Knowledge of change management and human-centered design; familiarity with advanced primary care concepts, chronic disease management, and FQHC/safety net clinics preferred
Excellent written and verbal communication skills
Strong facilitation skills, including the ability to design and lead effective meetings
