About the role

Key responsibilities & impact
  • Provide direct coaching support to clinical teams, both in-person and remotely
  • Assist care teams in obtaining and using data to create clinical/operational action plans
  • Support health center team adoption of technology solutions to drive practice transformation
  • Manage multiple concurrent clinical improvement projects
  • Identify, document and share best practice insights and actionable recommendations

Requirements

What you’ll need
  • Bachelor's degree in healthcare or related field, master's degree strongly preferred
  • Minimum of three (3) years of experience supporting health care teams in similar or relevant role strongly preferred
  • Strong process understanding of project lifecycle, highly independent and self-motivated
  • Demonstrated experience supporting individual clinicians, care teams, and clinic leaders with implementing practice changes in a clinical setting
  • Knowledge of change management and human-centered design; familiarity with advanced primary care concepts, chronic disease management, and FQHC/safety net clinics preferred
  • Excellent written and verbal communication skills
  • Strong facilitation skills, including the ability to design and lead effective meetings
