About the role
Key responsibilities & impact- Provide direct coaching support to clinical teams, both in-person and remotely
- Assist care teams in obtaining and using data to create clinical/operational action plans
- Support health center team adoption of technology solutions to drive practice transformation
- Manage multiple concurrent clinical improvement projects
- Identify, document and share best practice insights and actionable recommendations
Requirements
What you’ll need- Bachelor's degree in healthcare or related field, master's degree strongly preferred
- Minimum of three (3) years of experience supporting health care teams in similar or relevant role strongly preferred
- Strong process understanding of project lifecycle, highly independent and self-motivated
- Demonstrated experience supporting individual clinicians, care teams, and clinic leaders with implementing practice changes in a clinical setting
- Knowledge of change management and human-centered design; familiarity with advanced primary care concepts, chronic disease management, and FQHC/safety net clinics preferred
- Excellent written and verbal communication skills
- Strong facilitation skills, including the ability to design and lead effective meetings
