Lead strategic programs for equitable clinical, financial, and operational outcomes
Provide consultation and technical assistance
Manage grant compliance and foster positive funder relationships
Identify opportunities to improve adoption and implementation of tools

Requirements

What you’ll need

Bachelor's degree in healthcare or a related field, master's degree strongly preferred
Minimum of three (3) years of experience supporting health care teams in similar or relevant roles strongly preferred and four to five (4–5) years a plus
Demonstrated strength in anticipating and resolving problems including where little to no precedent may exist
Strong experience supporting individual clinicians, care teams, and clinic leaders in implementing practice changes
Excellent written and verbal communication skills
Knowledge of change management and human-centered design; familiarity with advanced primary care concepts preferred
Strong facilitation skills, including designing and leading effective meetings
