About the role

Key responsibilities & impact
  • Provide practice transformation support to Health Center Organizations (HCOs)
  • Establish and reach clinical, operational, and/or quality improvement goals
  • Adopt best practices and implement innovations
  • Improve the experience and delivery of care for both patients and clinicians
  • Manage multiple concurrent clinical improvement projects
  • Facilitate peer learning sessions, webinars, office hours, and technical briefings
  • Support health center team adoption of technology solutions

Requirements

What you’ll need
  • Master’s degree in healthcare or a related field strongly preferred
  • Minimum of three (3) years of experience supporting health care teams in similar or relevant role strongly preferred
  • Four to five (4–5) years a plus
  • Strong process understanding of project lifecycle
  • Highly independent and self-motivated
  • Experience supporting individual clinicians, care teams, and clinic leaders with implementing practice changes in a clinical setting
  • Knowledge of change management and human-centered design
  • Familiarity with advanced primary care concepts, chronic disease management, and FQHC/safety net clinics preferred
  • Strong facilitation skills, including designing and leading effective meetings
  • Experience in a telecommuter role strongly preferred, including use of electronic documentation and file management
  • Ability to travel to urban and rural locations up to 25-50%
