CCM Intake MA, Bilingual
NsightMedical Assistant supporting Chronic Care Management team at Nsight Health. Guiding patients through health assessments and coordinating care with nursing staff in a remote setting.
About the role
Key responsibilities & impact- Support CCM nursing staff by completing monthly patient assessments and conducting Chronic Care Planning Assessment (CCPA) calls.
- Complete delegated clerical and administrative activities on behalf of nursing staff.
- Document patient interactions, assessments, and intake in the EHR.
- Maintain compliance with company policies and applicable regulations.
- Coordinate communication between patients, caregivers, nursing staff, and providers.
- Perform other duties, as assigned.
Requirements
What you’ll need- Active MA license required.
- Strong communication and organizational skills.
- Bilingual Proficiency: Fluent in English and Spanish.
- Passionate about patient care, possessing the ability to relate with empathy and compassion.
- A passion for patient teaching is a must.
- Minimum fiber internet speed of 50 Mbps download / 10 Mbps upload.
- Hardwired internet connection required.
- Speed test submission required during the offer process.
- Private, HIPAA-compliant workspace.
