About the role
Key responsibilities & impact- Assess the needs of children, youth and families, formulating treatment plans including goals and service strategies.
- Meet with clients and/or their families to provide support & counselling, using evidence based/informed programs.
- Actively participate in the treatment planning process for clients including monitoring of each individual's progress toward the attainment of their identified goals.
- Initiate or attend case conference meetings as required.
- Provide follow-up service to discharged clients as needed.
- Assist client(s) in making referrals to community resources as needed.
- Act as case manager for assigned client(s).
- Provide consultation to New Path Staff, other agency personnel or the community.
- Function as a supportive member of the team.
Requirements
What you’ll need- Bachelor of Social Work (B.S.W)
- Minimum two years' experience working with children, youth and their families in a children's mental health or related service
- Experience/training in Single Session Counselling
- Experience working with evidence based/informed programs
- Youth and family focused; provides client and family centered approach to care.
- Meaningfully engages youth and families and collaborates to facilitate positive change.
- Aligns own behavior with the values, needs, priorities and goals of the organization.
- Takes personal responsibility for enhancing individual and team performance by promoting cooperation and commitment.
- Demonstrates strong engagement skills using a strengths-based approach and partners with client/family in treatment planning, goal setting and anticipated outcomes.
- Recognizes, initiates and adapts to changing conditions and situations in the workplace.
- Demonstrates awareness for the backgrounds, values, beliefs and traditions of staff, clients, families and communities we serve.
- Continuously assesses and challenges assumptions, personal unconscious bias and commits to understanding diverse perspectives, experiences and their intersectionality.
- Commits to ongoing training to build the knowledge, awareness and skills, that contribute to creating a culturally safe environment for staff, clients and families.
- Competency and effective use of database and Microsoft Office systems.
- Confident, articulate and professional when communicating verbally and in writing.
- French language is an asset.
