Licensed Physical Therapy Assistant, Care Navigator
Net HealthCare Navigator role to assist patients with home-exercise therapy at Limber Health. Providing remote monitoring and organizing caseloads for musculoskeletal health support.
About the role
Key responsibilities & impact- Provide remote monitoring services to help support patients
- Direct and assist users in therapeutic exercise
- Provide ongoing feedback to the evaluating clinician
- Maintain necessary records and notes while providing remote monitoring
- Organize and manage a caseload while staying efficient
Requirements
What you’ll need- 2+ years of experience in musculoskeletal health is a plus
- Graduated from an accredited physical therapy program
- Active PT license in a compact state
- Active PTA license in Michigan
- Tech Savvy - thrive in a multi-platform environment
- Excellent communication skills
- Strong relationship-building and listening skills
- Ability to multi-task and manage various types of users
- Independent and proactive work style
