About the role
Key responsibilities & impact- Support quality improvement, outreach, clinical operations, and population health initiatives
- Conduct patient outreach to identify and resolve barriers to care
- Communicate with care teams across the organization
- Lead or support clinical operations workflow development and iterative improvement
- Support Quality Improvement/Population Health Managers with meeting preparation, data collection and analysis, clinical operations activities, quality improvement initiatives, and workflow development
- Build and maintain relationships with key stakeholders across the enterprise
- Collaborate with stakeholders to improve patient care and outcomes
- Implement and monitor workflow and screening fidelity
- Oversee volunteers for outreach activities
- Use internal and external reports, including Reporting Workbench, to assess outreach needs and close care gaps
- Perform and document patient outreach according to NeighborHealth protocols
- Provide EMR and technology training and serve as a resident expert on assigned applications/tools
- Handle email correspondence and other non-call-related functions
- Navigate EPIC to handle calls, schedule/reschedule/cancel appointments, document and route messages, and manage refill requests and encounters
- Participate in required departmental and organizational meetings and activities
- Adhere to organizational and departmental policies and procedures
Requirements
What you’ll need- Knowledge of clinic operations and patient-facing care
- Ability and willingness to learn and apply population health and quality improvement principles
- Proficiency in EPIC, Word, Excel, Visio, and PowerPoint
- Maintain excellent punctuality and attendance
- Outstanding customer service skills
- Ability to independently solve problems or seek consultation when appropriate
- Knowledge of MyChart and patient access workflows
- Ability to recognize red-flag or urgent symptoms and determine when immediate nurse involvement is required
- Background check required before beginning employment
- Must complete E-Verify employment verification requirements
