About the role

Key responsibilities & impact
  • Perform and document duties assigned by the RN Case Manager
  • Observe, record, and report clients’ care, symptoms, and emotional and physical status
  • Provide compassionate support to clients, family members, and caregivers
  • Provide personal care, including bathing, back rubs, oral hygiene, hair care, skin care, dressing and undressing, and toileting assistance
  • Process client laundry, change bed linen, and keep living areas clean and orderly
  • Plan and prepare nutritious meals and assist with feeding when necessary
  • Take vital signs when ordered
  • Assist with ambulation and transfers according to the plan of care
  • Perform range-of-motion exercises and other simple procedures as an extension of therapy services when ordered
  • Meet client safety needs and use equipment safely and properly

Requirements

What you’ll need
  • High school diploma or GED
  • Home Health Aide Certification training
  • Minimum of 12 months as an aide in a facility, home health/hospice agency, or acute care setting
  • STNA strongly preferred
  • Hospice experience preferred
  • Ability to perform personal care, meal preparation, feeding assistance, ambulation, transfers, and range-of-motion procedures
  • Ability to take vital signs when ordered
  • Ability to use equipment safely and properly
  • Availability Monday-Friday, 8 hours a day; possible weekends
