About the role
Key responsibilities & impact- Serve as a liaison between healthcare and housing professionals, including service coordinators, discharge planners, and community-based service providers.
- Maintain knowledge of all service lines: adult day health, personal care/private duty, service coordination, skilled care, and hospice.
- Collaborate closely with individual case managers to arrange services that align with each consumer’s preferences and community resources, including physicians, pharmacies, and other essential providers.
- Assist individuals in locating housing and obtaining necessary materials and supplies (i.e. social security card, ID, completing applications, etc.)
- Support consumers in accessing entitled benefits and resources.
- Provide exceptional customer service by addressing questions, concerns, and problems, ensuring daily needs are met.
Requirements
What you’ll need- High school diploma or GED required.
- Minimum of 1 year in social service or healthcare (STNA) delivery with adults, elderly and family populations; 2 years' experience preferred.
- Knowledge of supportive services and community resources in assigned areas/counties.
- Knowledge of housing services in the areas preferred.
- Valid driver’s license and compliance with National Church Residences motor vehicle policy is required.
