Registered Nurse Case Manager

About the role
Key responsibilities & impact- Provide care for hospice patients in their homes.
- Coordinate patient care with interdisciplinary team members.
- Participate in an all-call weekend rotation.
- Serve families by providing education about hospice services.
Requirements
What you’ll need- Degree in nursing field as a registered nurse from an approved school of nursing.
- Minimum of two years as an RN.
- Home health/hospice experience preferred.
