About the role
- Evaluate and develop individualized treatment plans to restore function, improve mobility, and relieve pain
- Provide direct therapy services in the home setting, helping residents achieve their highest level of independence
- Collaborate with the interdisciplinary team to ensure coordinated and comprehensive care
- Document patient progress and outcomes accurately and timely
- Educate patients, families, and caregivers on therapy techniques and home exercise programs
Requirements
- Current state licensure as a Physical Therapist (PT)
- Graduate of an accredited Physical Therapy program
- Home health experience preferred, but not required
- Compassion, adaptability, and a passion for serving seniors
