Oversee clinical operations within the Greater LA Market, leading clinic, home care, and complex care nurses and day center LVNs
Serve as a member of the MyPlace Health leadership team to drive policy development, census growth, staff development, and participant outcomes
Lead recruitment, onboarding, training, coaching, and performance evaluations for clinical staff (RNs and Day Center LVNs)
Oversee Home Care Team operations, support the Home Care Supervisor, and manage partnerships with home care vendors
Develop, update, and implement clinical policies and procedures aligned with best practices, MyPlace Health policies, and PACE & CMS regulations
Collaborate with the Quality Team to conduct regular audits and reviews and implement continuous improvement initiatives
Partner with the Medical Director and interdisciplinary teams to deliver coordinated, participant-centered care and communicate treatment plans to participants and families
Provide regular performance and clinical outcome reports to leadership
Lead and supervise the interdisciplinary Fall Prevention Program to ensure participant safety
Take on additional tasks and responsibilities as needed to support the team and operations
Requirements
Bachelor of Science in Nursing (BSN) required
Master’s degree in management or a healthcare-related field is a plus
Active RN license in the state of California
BLS Certification (or willingness to obtain)
3-5 years of clinical experience, including at least 2 years in a leadership or management role
Additional certifications in management, leadership, or quality assurance are a plus
Strong leadership and mentorship abilities
Collaborative spirit and ability to work with interdisciplinary teams
Passion for working with older adults
Deep knowledge of healthcare regulations, compliance standards, and best practices (including PACE & CMS regulations)
Experience in clinical operations, resource allocation, and performance management
Ability to problem-solve, think critically, and implement process improvements
Excellent communication, organizational, and time management skills
Ability to work onsite Monday - Friday 8:00 am - 5:00 pm
