Oversee day-to-day clinical operations at assigned center(s) as the accountable medical leader
Deliver evidence-based, high-quality, and cost-effective medical services to PACE participants
Provide comprehensive medical care including evaluations, diagnoses, treatment plans, and coordination with specialists and hospital teams
Maintain meticulous, clear, and compliant medical documentation
Recruit, train, supervise, and mentor physicians, advanced practitioners, and transitions of care coordinators
Train and oversee contracted primary care physicians, advanced practitioners, and specialists
Oversee development and implementation of the Quality Improvement and Performance Improvement Program (QAPI)
Monitor utilization trends (inpatient hospitalizations, ER visits) and set outcome targets
Design and implement interventions to improve participant health, quality of life, and satisfaction
Oversee periodic chart audits for quality, regulatory compliance, utilization, medication practices, and risk areas
Ensure adequate primary care staffing and oversee after-hours on-call arrangements for 24/7 provider availability
Provide consultation on grievances, appeals, and IDT decision-making; evaluate medical-related grievances as needed
Monitor quality of care provided by contracted partners (hospitals, SNFs, long-term care facilities)
Build collaborative relationships with community physicians and providers to foster referrals
Promote teamwork, accountability, and professional development of PACE primary care staff through meetings and coaching
Participate in Quality Committee and other committees or workgroups
Perform additional duties within the scope of professional license as assigned
Requirements
MD or DO required
Active board certification in geriatrics, complex care, home-based care, palliative care, internal medicine, family medicine, or related clinical area (and maintain certification throughout employment)
At least five years practicing medicine
Minimum of two years working directly with elderly or frail populations
Previous experience in a PACE model or similar integrated care environments (preferred)
Current, unrestricted medical license in the states where we operate
Current DEA certificate (credentialed to prescribe)
Up-to-date immunization records
Bilingual abilities (English and Spanish) preferred
Ability to recruit, train, supervise, and mentor physicians and advanced practitioners
Must be 100% present at clinic, in community, or visiting participants in their homes as needed
