About the role
- Providing direct support to clients, including assistance with paperwork, goal planning, and skill-building.
- Maintaining accurate and timely client documentation, including dates, times, and progress notes for all meetings, in the electronic health record system.
- Building rapport with clients while maintaining professional boundaries at all times
- Providing services primarily in the community or clients’ homes
- Providing staff with client history/special requirements and hosting team meetings
- Making referrals to other services such as mental health services, parenting, etc.
- Communicating with clients’ therapists for care coordination and treatment planning
- Providing transportation and serving as an advocate for clients at appointments
- Completing and overseeing the discharge process for all assigned clients
- Meeting and maintaining billing hours expectations and documentation timelines
- Attending meetings and training as required and meeting regularly with their supervisor
- Other appropriate and job-related tasks as assigned
Requirements
- Bachelor’s degree in social work, psychology, rehabilitation counseling, education, vocational rehabilitation, or a similar field and/or related experience
- At least 2-3 years of experience in supported employment, vocational rehabilitation, mental health services, or related fields.
- Experience working with individuals with mental health conditions, disabilities, or other barriers to employment is strongly preferred.
- Familiarity with the Individual Placement and Support (IPS) model or similar evidence-based employment programs is preferred.
- Certification in Supported Employment (CESP) or related credentials are a plus
- CPR/First Aid certification (or willingness to obtain it within a designated time frame) is preferred.
- Any state-specific licensure or certification (e.g., Licensed Professional Counselor, Licensed Social Worker) is preferred but not required, depending on program needs.
- Familiar with social services in the Missoula community
- Experience providing services to young adults with special needs, severe emotional disturbances, and/or cognitive or developmental delays.
