Key responsibilities & impact

Manage performance against clinical and operational metrics
Lead, motivate, and drive accountability for a team of nurse practitioners, registered nurses, and social workers
Prepare, review and analyze performance and health outcome reports
Plan staffing schedules to optimize efficiency and productivity
Assist in the hiring of clinical staff members
Execute corporate and regional strategy to deliver quality patient care
Perform other duties as assigned

Requirements

What you’ll need

Experience managing a high-risk patient population
Experience leading clinical staff in a care management, managed care and/or home health environment
Microsoft Excel and Salesforce experience preferred
Experience analyzing clinical and operational performance
Basic Life Support (BLS) certification required
BS/BA or equivalent experience
3+ years previous experience working in Managed Care or Delegated Risk Partnerships
Bilingual Preferred
