Health Navigator, Monitoring
ModivcareHealth Navigator engaged in daily interaction with clients, monitoring health alerts, and managing inbound/outbound calls in a remote capacity. Seeking professionals with medical experience for impactful roles.
About the role
Key responsibilities & impact- Engaging daily with clients and teammates to build and sustain positive and productive working relationships.
- Monitoring device readings, contacting clients to determine if they require further intervention or assistance.
- Managing inbound calls and handling all incoming alarm signal activities.
- Making outbound calls to reach customers, emergency contacts, and Police/Fire/EMS regarding alarm activities.
- Following standard operating procedures.
- Verifying demographics and other related data and entering patient information into appropriate systems.
- Answering routine inquiries and relaying information and messages to appropriate health professionals, agencies, departments, and other individuals or groups requiring notification.
- Maintaining access to, and security of, sensitive materials.
- Explaining products or services and answering questions from customers.
- Maintaining accurate records of contacts, accounts, and orders.
- Troubleshooting devices and reporting malfunctions to ensure units receive necessary maintenance.
Requirements
What you’ll need- High School Diploma required.
- Two (2) plus years of related professional or healthcare medical experience.
- LPN/CNA/Medical Assistant certification preferred.
- Call Center experience preferred.
- Or equivalent combination of education and/or experience.
- Strong analytical and problem-solving skills.
- An energetic pace along with a positive attitude.
- Self-motivation and the ability to work independently and with teams.
- Proficient in the use of Microsoft Teams, Word, Excel, and Outlook.
- Excellent oral and written communication skills.
- Proficient in managing multiple tasks at the same time.
