About the role
- Conduct biopsychosocial assessments for members
- Provide individual counseling and group therapy with adolescents and adults
- Design and lead therapy groups based on interests and evidence-based practices
- Collaborate with member, family and healthcare providers to develop individualized plans of care
- Identify and initiate referrals for social service programs (financial, psycho-social, community and state supportive services)
- Advocate for members and families to ensure needs and choices are represented
- Utilize approved clinical criteria to assess and determine appropriate level of care
- Document all member assessments, care plans and referrals provided
- Work with team to coordinate care across substance use disorder treatment settings
- Responsible for achieving set goals and Key Performance Indicators (KPIs)
- Learn the StrongWell model and take ownership for outcome-based care
- Participate in interdisciplinary team meetings and utilization management rounds
- Maintain excellent punctuality and attendance during work hours
- Promote responsible and ethical stewardship of company resources
Requirements
- Current license: LCSW, LMFT, LPC, or LMHC in Tennessee, Kentucky, Louisiana, or Washington State
- Preferred experience with substance abuse population
- Background in maternal; substance abuse preferred
- Experience with adults and adolescents
- Familiarity with Medicare and Medicaid procedures
- Remote Work Experience
- Willingness to obtain additional state licensure (Mindoula covers Washington licensure costs)
- Therapists supporting Washington-based members expected to work Pacific Standard Time (PST) hours, typically 9:00 AM to 5:00 PM PST
