About the role
Key responsibilities & impact- Supervise assigned home care personnel and field LVNs/LPNs/RNs
- Enforce and demonstrate compliance with State and Federal regulations
- Coordinate implementation of each patient’s Plan of Care
- Instruct, train, and re-educate staff on clinical skills and procedures
- Provide support, guidance, and education to patients, families, and caregivers
- Communicate occurrences and pertinent information to clinical leadership
- Report suspected child or adult abuse, neglect, or exploitation to proper authorities
- Collaborate with clinical leadership to resolve client and clinical concerns
- Develop and implement treatment plans under physician direction
- Complete and review clinical documentation, admissions, recertifications, and resumptions of care
- Interview and evaluate clinical field staff for competency and safe patient care
- Participate in quality improvement, incident reporting, infection reporting, and risk management
- Make patient and personnel assignments
- Coordinate patient care and referrals
- Continually assess patient needs
- Ensure development, implementation, and updating of individualized plans of care
- Support and execute the company mission, ethics, and goals
Requirements
What you’ll need- Current state license as a Registered Nurse
- Minimum of two years of nursing experience in a Home Health or Acute setting preferred
- BSN preferred
- Leadership capabilities with strong supervisory and interpersonal skills
- Detail-oriented, flexible, organized, and capable of effective time management
- Knowledge of cost management for clinical services
- Current CPR card
- Current health certificate as applicable
- Current state driver’s license and automobile insurance
