Alliance Liaison, RN – Community

About the role
Key responsibilities & impact- Enhances continuity of patient care by providing collaboration between hospital, physicians, hospice/home care, and caregivers
- Works with case management, social workers, and others on health care team to transition patient to post-acute care setting
- Responsible for building and maintaining partnerships that enhance post-acute care referrals to Alliance
- Collaborates to develop and execute market strategies, improve service offerings, and implement initiatives that support improved patient care and growth in division sales
- Ensures documentation is complete by reviewing patient orders, verifying insurance information, gathering clinical data and determining patient care needs
- Facilitates the transfer of patients from hospital to home care/hospice services and consults with nursing, social services and other support services
Requirements
What you’ll need- Required - Associates Degree Nursing
- Required - Registered Nurse 3-5 years
- Preferred - Bachelor's Degree Nursing
- Required - Registered Nurse Tennessee - Tennessee Board of Nursing
- Required - BASIC LIFE SUPPORT - American Heart Association
- Preferred: Certification in clinical specialty
