About the role

Key responsibilities & impact
  • Receive and review all incoming BluePrint referrals from employees, supervisors, Human Resources, or other approved referral sources.
  • Create and maintain employee records in the case management system, Click, upon receipt of a referral.
  • Initiate contact with referred employees within established program timeframes.
  • Conduct onsite and virtual initial meetings to introduce the BluePrint Program and explain available services, confidentiality, voluntary participation, and program expectations.
  • Ensure employees have a clear understanding of the program to support informed decision-making regarding enrollment.
  • Facilitate completion of all required enrollment and consent documentation for employees who elect to participate.
  • Complete the Pathways Assessment for all enrolled referrals to identify employee needs, barriers, strengths, and goals.
  • For employees who decline enrollment, provide appropriate resource navigation and referrals to internal and external supports based on identified needs, including community resources, benefits, and wellness services.
  • Prepare completed intake files and conduct a warm handoff to the assigned BluePrint Specialist to ensure continuity of care and seamless service delivery.
  • Close referrals appropriately when employees decline participation, are ineligible, or cannot be engaged after documented outreach efforts.
  • Ensure all referrals are transferred or closed within 30 calendar days of the referral date.
  • Log all activities, assessments, referrals, and time spent in the case management system within required timeframes.
  • Maintain up-to-date knowledge of internal and external resources and guide employees in accessing these supports.
  • Maintain strict confidentiality protocols, only disclosing information under legally or ethically mandated circumstances.
  • Reinforce employee autonomy and ensure all participation is voluntary.
  • Coordinate regularly with managers to maintain alignment with staffing schedules and ensure availability during employee shifts.
  • Participate in program evaluation efforts by capturing impact data and contributing to outcome reporting.
  • Participate in team meetings, trainings, and program improvement efforts.
  • Follow and enforce all MERS/Goodwill policies and procedures.
  • All other duties as assigned.

Requirements

What you’ll need
  • High school diploma or equivalent is required.
  • Bachelor’s degree in relevant field preferred.
  • Two years’ experience in intake, case management, or workforce development required.
  • Experience conducting assessments and developing service plans.
  • Experience using electronic case management systems and maintaining accurate documentation.
  • Demonstrated proficiency in Microsoft applications required; case management software experience preferred.
  • Valid state driver’s license, reliable transportation, and current auto insurance required.
  • Must be able to travel within territory (Missouri, Southern Illinois, and Eastern Kansas).
  • Ability to obtain/maintain certifications is required, such as Mental Health First Aid and Financial Health Counselor.
