About the role

Key responsibilities & impact

Finding resources and services for clients that best fit their needs
Advocating for persons with disabilities/elderly
Carrying caseloads consisting of persons with intellectual disabilities, traumatic brain injuries, CADI diagnoses, and elderly/alternative care
Visiting persons on the caseload and attending team meetings
Completing needs assessment; individual service plan development (CSSP)
Assisting with planning for new service development
Locating residential, vocational, and other needed services
Monitoring service delivery and ensuring the health and safety needs of each person are being met
Completion of referrals and crisis intervention as needed
Developing waiver budgets
Completing case notes

Requirements

What you’ll need

Be a licensed Social Worker (Bachelor of Social Work required for licensure)
Or licensed Graduate Social Worker (Master’s in Social Work required for licensure) as stated by the Minnesota Board of Social Work
Or have a 4-year degree in Human Services, Psychology or Sociology, or related fields
Successful clearance of Department of Human Services background check
Successful clearance of Motor Vehicle Background Check and acceptable driving record per Company Policy
