About the role
Key responsibilities & impact- Finding resources and services for clients that best fit their needs
- Advocating for persons with disabilities/elderly
- Carrying caseloads consisting of persons with intellectual disabilities, traumatic brain injuries, CADI diagnoses, and elderly/alternative care
- Visiting persons on the caseload and attending team meetings
- Completing needs assessment; individual service plan development (CSSP)
- Assisting with planning for new service development
- Locating residential, vocational, and other needed services
- Monitoring service delivery and ensuring the health and safety needs of each person are being met
- Completion of referrals and crisis intervention as needed
- Developing waiver budgets
- Completing case notes
Requirements
What you’ll need- Be a licensed Social Worker (Bachelor of Social Work required for licensure)
- Or licensed Graduate Social Worker (Master’s in Social Work required for licensure) as stated by the Minnesota Board of Social Work
- Or have a 4-year degree in Human Services, Psychology or Sociology, or related fields
- Successful clearance of Department of Human Services background check
- Successful clearance of Motor Vehicle Background Check and acceptable driving record per Company Policy
