About the role

Key responsibilities & impact

Provide cardiac rehabilitation services through patient assessment, education, treatment planning, and supervised exercise sessions
Coordinate referrals, intake scheduling, and new patient orientations while developing individualized care plans
Serve as a case manager for assigned patients, monitoring progress, modifying treatment plans as needed
Track outcomes and coordinate care with physicians and other healthcare professionals
Monitor patient safety during rehabilitation sessions, including vital signs, EKGs, symptoms, and blood glucose testing when appropriate
Facilitate individual and group education programs focused on cardiovascular health and lifestyle modification
Support related wellness initiatives, including weight management services, fitness assessments, and community outreach

Requirements

What you’ll need

Bachelor's Degree in Exercise Physiology or related field required
Master's Degree in Exercise Physiology preferred
Certification through ACSM or AACVPR required within 3 years if no Master's Degree
Current BLS certification
Advanced Cardiac Life Support (ACLS) preferred
Interpretive/analytical skills
Effective interpersonal communication skills
Organizational skills
Microsoft Office software knowledge
