About the role

Key responsibilities & impact

Provide virtual care to patients that benefit from ongoing chronic disease management
Monitor vital sign trends
Conduct patient visits in between regular office visits
Assist in panel management of patients
Provide individualized care to improve patient outcomes
Build strong patient relationships
Support chronic disease management and acute care needs
Facilitate advance care planning for Medicare and MA patients
Collaborate with primary care clinicians to coordinate care
Perform virtual assessments, patient education, triage, and escalation of care needs
Document all care provided and maintain patient confidentiality

Requirements

What you’ll need

Active RN license in applicable state(s)
Experience in chronic disease management and virtual care preferred
Strong communication and collaboration skills
Commitment to Mercy’s Nursing Conceptual Framework and patient-centered care
