About the role
Key responsibilities & impact- Support physicians and interdisciplinary teams in facilitating patient transitions from hospital to home with home health services
- Provide timely and accurate information to physicians, Case Managers, patients, families and payors
- Collaborate with physicians, nursing staff, multidisciplinary care teams, patients, families and Home Health companies
- Identify and resolve discharge delays and obstacles involving ordered durable medical equipment
- Facilitate patient care, monitor progress to discharge and intervene as necessary
- Communicate and document plan-of-care elements to ensure continuity of care
- Document Home Health durable medical equipment information in medical records
- Collect and inform patients and families of co-pays and process monies collected
- Demonstrate proper use of durable medical equipment and coordinate delivery to patients’ homes
- Maintain appropriate stock in the consignment closet
- Adhere to Memorial Hermann policies, procedures, standards, budgetary specifications, safety, time management, supply management, productivity and quality-of-service requirements
- Meet mandatory/continuing education and skills competency requirements
- Support department goals and serve as preceptor, mentor and resource to less experienced staff
- Provide compassionate, personalized, safe and efficient experiences to patients and colleagues
- Perform other duties as assigned
Requirements
What you’ll need- High School Diploma preferred
- Two (2) years of previous healthcare experience
- Strong analytical, data management and computer skills
- Current working knowledge of discharge planning, Medicare, Medicaid and managed care reimbursement
- Knowledge of rules and regulations governing Home Health
- Ability to work independently and exercise sound judgment in interactions with physicians, payors and patients and their families
