About the role

Key responsibilities & impact

Support physicians and interdisciplinary teams in facilitating patient transitions from hospital to home with home health services
Provide timely and accurate information to physicians, Case Managers, patients, families and payors
Collaborate with physicians, nursing staff, multidisciplinary care teams, patients, families and Home Health companies
Identify and resolve discharge delays and obstacles involving ordered durable medical equipment
Facilitate patient care, monitor progress to discharge and intervene as necessary
Communicate and document plan-of-care elements to ensure continuity of care
Document Home Health durable medical equipment information in medical records
Collect and inform patients and families of co-pays and process monies collected
Demonstrate proper use of durable medical equipment and coordinate delivery to patients’ homes
Maintain appropriate stock in the consignment closet
Adhere to Memorial Hermann policies, procedures, standards, budgetary specifications, safety, time management, supply management, productivity and quality-of-service requirements
Meet mandatory/continuing education and skills competency requirements
Support department goals and serve as preceptor, mentor and resource to less experienced staff
Provide compassionate, personalized, safe and efficient experiences to patients and colleagues
Perform other duties as assigned

Requirements

What you’ll need

High School Diploma preferred
Two (2) years of previous healthcare experience
Strong analytical, data management and computer skills
Current working knowledge of discharge planning, Medicare, Medicaid and managed care reimbursement
Knowledge of rules and regulations governing Home Health
Ability to work independently and exercise sound judgment in interactions with physicians, payors and patients and their families
