About the role
Key responsibilities & impact- Review medical record documentation to identify pertinent diagnoses and procedures requiring code assignment for outpatient records
- Accurately code diagnoses and procedures using ICD-10-CM coding conventions for reimbursement, research, and federal regulatory compliance
- Review records to assure diagnosis and procedure specificity and appropriate reimbursement for hospital and professional charges
- Query physicians when code assignments are not straightforward or documentation is inadequate, ambiguous, or unclear
- Keep abreast of coding and reimbursement reporting guidelines and bring concerns to the manager
- Assign ICD-10-CM, modifiers, and HCPCS codes to outpatient records
- Maintain 95% or greater ICD-10-CM and/or HCPCS coding accuracy and meet productivity standards while minimizing abstracting errors
- Abide by AHIMA Standards of Ethical Coding and official coding guidelines
- Ensure safe care and adhere to Memorial Hermann policies, procedures, standards, budgetary specifications, and quality expectations
- Support department goals and serve as a preceptor, mentor, and resource to less experienced staff
- Create compassionate and personalized experiences for patients and colleagues
- Perform other duties as assigned
Requirements
What you’ll need- High School Diploma or GED required
- Associate degree in Health Information Management or any Healthcare Related Field preferred
- RHIT, RHIA, or relevant coding certification(s) from AHIMA or AAPC required
- Three years of hospital or ambulatory coding experience required for Surgery/Observation
- Two years of hospital or ambulatory coding experience required for Emergency Department
- Two years of hospital or ambulatory coding experience required for Interventional Radiology
- One year of hospital or ambulatory coding experience preferred for Ancillary/Recurring
- Experience coding in a level 1 trauma facility/academic teaching facility preferred
- Effective oral and written communication skills
- Strong knowledge of ICD-10-CM and/or CPT
- Analytical skills to interpret health record data and assign appropriate codes
- Proficient knowledge of human anatomy, physiology, medical terminology, and surgical terminology
- Critical thinking, good judgment, and decision-making skills
- Knowledge of coding compliance policies, official coding guidelines, regulatory requirements, and internal policies and procedures
- Proficient navigation of a Windows-based application environment
