About the role

Key responsibilities & impact
  • Participate in a structured six (6) month rotational program across various functions of the Community Health and Community Benefits team
  • Gain exposure to best practices in patient engagement, community resources, and organizational practices
  • Conduct intake, needs & eligibility assessments, and referrals of patients under staff supervision
  • Complete a project and/or presentation at end of rotation that demonstrates knowledge obtained during experience
  • Ensure safe care to patients, staff and visitors

Requirements

What you’ll need
  • High school diploma or GED preferred
  • Community Health Worker Certification preferred
  • Effective oral and written communication skills
  • Entry-level computer software skills – Microsoft Word, Excel, and PowerPoint
  • Strong coordination and organizational skills
  • Ability to work collaboratively in a fast-paced, service-oriented environment
