About the role
Key responsibilities & impact- Participate in a structured six (6) month rotational program across various functions of the Community Health and Community Benefits team
- Gain exposure to best practices in patient engagement, community resources, and organizational practices
- Conduct intake, needs & eligibility assessments, and referrals of patients under staff supervision
- Complete a project and/or presentation at end of rotation that demonstrates knowledge obtained during experience
- Ensure safe care to patients, staff and visitors
Requirements
What you’ll need- High school diploma or GED preferred
- Community Health Worker Certification preferred
- Effective oral and written communication skills
- Entry-level computer software skills – Microsoft Word, Excel, and PowerPoint
- Strong coordination and organizational skills
- Ability to work collaboratively in a fast-paced, service-oriented environment
