About the role
Key responsibilities & impact- Review medical record documentation to identify diagnoses and procedures requiring code assignment for outpatient records
- Accurately code diagnoses and procedures using ICD-10-CM conventions for reimbursement, research and compliance
- Review records for diagnosis and procedure specificity and appropriate reimbursement for hospital and professional charges
- Query physicians when assignments are unclear or documentation is inadequate or ambiguous
- Keep current with coding and reimbursement reporting guidelines and escalate concerns to the manager
- Assign ICD-10-CM, modifiers and HCPCS codes to outpatient records
- Maintain 95% or greater ICD-10-CM and/or HCPCS coding accuracy and meet productivity standards
- Minimize abstracting errors
- Follow AHIMA Standards of Ethical Coding and official coding guidelines
- Adhere to Memorial Hermann policies, procedures and service standards
- Support mandatory education, competency requirements and department goals
- Serve as preceptor, mentor and resource to less experienced staff
- Create compassionate, personalized and efficient experiences for patients and colleagues
- Perform other duties as assigned
Requirements
What you’ll need- High School Diploma or GED required
- RHIT, RHIA, or relevant AHIMA/AAPC coding certification(s) required
- Surgery/Observation: three (3) years of hospital or ambulatory coding experience required
- Emergency Department: two (2) years of hospital or ambulatory coding experience required
- Interventional Radiology: two (2) years of hospital or ambulatory coding experience required
- Ancillary/Recurring: one (1) year of hospital or ambulatory coding experience preferred
- Experience coding in a level 1 trauma facility/academic teaching facility preferred
- Effective oral and written communication skills
- Strong knowledge of ICD-10-CM and/or CPT
- Analytical skills for interpreting health records and assigning appropriate codes
- Proficient knowledge of human anatomy, physiology, medical terminology and surgical terminology
- Critical thinking, good judgment and decision-making skills
- Knowledge of coding compliance policies, official coding guidelines, regulatory requirements and internal policies and procedures
- Proficient in navigating a Windows-based application environment
- If located outside Texas, must be located in Kansas, Oklahoma, Tennessee, Louisiana, Georgia, South Carolina, or North Carolina, or willing to relocate to an eligible state including Texas
