About the role

Key responsibilities & impact

Review and abstract inpatient medical records
Identify and accurately assign diagnosis, procedure and service codes in inpatient settings
Collect and include data elements required for federal or state reporting and billing
Review documentation for pertinent diagnoses and procedures requiring code assignment
Code diagnoses and procedures using ICD-10 conventions for reimbursement, research and regulatory compliance
Review records for diagnostic and procedural specificity and appropriate reimbursement
Query physicians when documentation is inadequate, ambiguous, unclear or coding assignments are not straightforward
Monitor coding and reimbursement reporting guidelines and escalate concerns to the manager
Assign ICD-10 codes and DRGs to inpatient records
Maintain 92%–95% accuracy in ICD-10 and DRG assignment and meet productivity standards while minimizing abstracting errors
Follow AHIMA Standards of Ethical Coding and official coding guidelines
Adhere to Memorial Hermann policies, procedures and service standards
Complete mandatory/continuing education and skills competency requirements
Support department goals and mentor less experienced staff
Perform other duties as assigned

Requirements

What you’ll need

High School Diploma or GED required
Associate degree in Health Information Management or any Healthcare Related Field preferred
One of RHIT, RHIA, CCA, CCS, or CIC coding certifications from AHIMA or AAPC required
Five (5) years of inpatient hospital coding experience required
Experience coding in a level 1 trauma facility/academic teaching facility strongly preferred
Effective oral and written communication skills
Strong knowledge of ICD-10-CM and PCS coding
Analytical skills to interpret health-record data and assign appropriate codes
Strong knowledge of human anatomy, physiology, medical terminology and surgical terminology
Critical thinking, judgment and decision-making skills
Knowledge of coding compliance policies, official coding guidelines, regulatory requirements and internal coding procedures
Proficiency navigating a Windows-based application environment
Must be located in Texas, Kansas, Oklahoma, Tennessee, Louisiana, Georgia, South Carolina, or North Carolina, or be willing to relocate to one of these eligible states
