About the role
- Provide mobile clinical care to Australians in their homes
- Support Home Care Package clients
- Focus on Rehab in the Home program
- Complete assessments for Residential Aged Care clients
- Develop and manage programs and facilitate group programs
- Complete individual sessions with residents
- Prescribe equipment and facilitate manual handling
Requirements
- Fully AHPRA registered
- Minimum 2 years’ experience in a Community or Aged Care setting
- Comfortable travelling to clients located across Adelaide and provide your own vehicle
- Experience in providing home assessments; equipment prescriptions; and home modifications
- Experience in supporting post orthopaedic surgery and clients after hospitalisation
- Experience in completing assessments, developing, implementing, and managing programs in Aged Care
