About the role
- Deliver services to Home Care Package clients, including home assessments, equipment prescription, and home modifications
- Support recovery through Rehab in the Home (post-orthopaedic surgery)
- Assist clients post-hospitalisation via the Hospital in the Home program
Requirements
- Fully AHPRA registered
- New grads welcome; experience in community or aged care is a plus
- Skilled in promoting health and wellbeing through client-centred care
