About the role
Key responsibilities & impact- Conduct comprehensive intake assessments to identify medical, behavioral, social, and environmental needs.
- Develop individualized care plans that support healthy pregnancies and positive birth outcomes.
- Coordinate care with physicians, hospitals, specialists, behavioral health providers, and community agencies.
- Conduct home visits, virtual visits, and telephone follow-ups to assess member needs and monitor progress.
- Educate members on prenatal care, postpartum recovery, infant care, breastfeeding, safe sleep, nutrition, and maternal wellness.
- Connect families with community resources, including transportation, housing assistance, food programs, childcare resources, behavioral health services, and financial assistance programs.
- Advocate for members to reduce barriers to healthcare and improve access to needed services.
- Monitor member progress and adjust care plans as needed throughout pregnancy and the postpartum period.
- Maintain accurate documentation within electronic health record systems and care management platforms.
- Collaborate with interdisciplinary teams to promote coordinated, culturally responsive care.
- Participate in quality improvement initiatives and community outreach activities.
- Maintain compliance with all state, federal, Medicaid, HIPAA, and organizational requirements.
Requirements
What you’ll need- Bachelor's degree in Social Work, Public Health, Human Services, Nursing, Psychology, Sociology, Early Childhood Education, or a related field.
- Minimum of two (2) years of experience in case management, care coordination, social services, healthcare, behavioral health, community health, or a related field.
- Valid driver's license, reliable transportation, and automobile insurance.
- Ability to travel throughout assigned service areas.
- Strong organizational, communication, and documentation skills.
- Ability to maintain confidentiality and comply with HIPAA requirements.
- Experience in maternal health, prenatal care, postpartum care, or women's health preferred.
- Knowledge of Medicaid managed care or care coordination programs preferred.
- Community Health Worker (CHW) experience preferred.
- Experience serving high-risk or underserved populations preferred.
- Bilingual (English/Spanish) strongly preferred.
