About the role
- Rapidly identify, assess, and resolve disruptions or gaps in clinical reasoning across assigned solutions
- Document triage events, interventions, and outcomes in designated tracking systems
- Monitor the flow and coherence of clinical reasoning across care teams, ensuring seamless care delivery
- Identify patterns of reasoning breakdown and translate insights into actionable improvement strategies
- Analyze clinical reasoning trends across customer accounts and report findings to leadership
- Partner cross-functionally to address findings and implement solutions
- Model a culture of intellectual curiosity, clinical excellence, and continuous improvement
- Stay current on evidence-based practices, clinical reasoning research, and emerging care delivery trends
Requirements
- Clinically experienced professional with a background in nursing or a related healthcare discipline
- Experience in case management or care coordination within a health plan or hospital setting
- Strong familiarity applying MCG guidelines in real-world clinical practice
- Highly analytical thinker with the ability to synthesize complex clinical, operational, and relational data
- Adaptable and resilient, with the ability to thrive in fast-paced, ambiguous environments
- Detail-oriented with strong problem-solving skills and the ability to quickly assess and respond to issues
- Effective communicator and collaborator who works well across cross-functional teams
- Proficient in Microsoft Office Suite
- Active RN License Required.
