About the role
Key responsibilities & impact- Complete routine medical and/or behavioral health assessments using standardized tools and criteria
- Escalate complex or non-standard cases outside defined protocols
- Conduct on-site, video, and/or telephonic assessments
- Determine individuals’ service and support needs, eligibility, and level of care
- Perform duties in compliance with Person First standards, HIPAA guidelines, and confidentiality policies
- Ensure accurate and consistent documentation of assessment outcomes
- Travel to client, consumer, or program locations to perform in-person assessments exceeding 50% of the workweek
- Evaluate individuals in homes, group homes, and care facilities to determine long-term care needs and appropriate placement options
- Support Maximus’s Statewide Assessing Services program for Maine’s Department of Health and Human Services
Requirements
What you’ll need- Specific clinical license and experience requirements based on contract requirements
- Minimum two (2) years of post-graduate nursing experience
- Ability to travel on-site to client, consumer, or program locations for more than 50% of each workweek
- Ability to commute within and cover Cumberland & York County, Maine
- Reliable high-speed internet: minimum 20 Mbps download / 50 Mbps for shared internet connectivity and minimum 5 Mbps upload
- Private and secure workspace
- Experience in long-term care or home health care preferred
- Experience with OASIS or MDS preferred
