About the role

Key responsibilities & impact

Complete routine medical and/or behavioral health assessments using standardized tools and criteria
Escalate complex or non-standard cases outside defined protocols
Conduct on-site, video, and/or telephonic assessments
Determine individuals’ service and support needs, eligibility, and level of care
Perform duties in compliance with Person First standards, HIPAA guidelines, and confidentiality policies
Ensure accurate and consistent documentation of assessment outcomes
Travel to client, consumer, or program locations to perform in-person assessments exceeding 50% of the workweek
Evaluate individuals in homes, group homes, and care facilities to determine long-term care needs and appropriate placement options
Support Maximus’s Statewide Assessing Services program for Maine’s Department of Health and Human Services

Requirements

What you’ll need

Specific clinical license and experience requirements based on contract requirements
Minimum two (2) years of post-graduate nursing experience
Ability to travel on-site to client, consumer, or program locations for more than 50% of each workweek
Ability to commute within and cover Cumberland & York County, Maine
Reliable high-speed internet: minimum 20 Mbps download / 50 Mbps for shared internet connectivity and minimum 5 Mbps upload
Private and secure workspace
Experience in long-term care or home health care preferred
Experience with OASIS or MDS preferred
