About the role
Key responsibilities & impact- Conduct person-centered medical and/or behavioral health assessments using standardized tools and criteria
- Escalate complex or non-standard cases outside defined protocols
- Conduct on-site, video, and/or telephonic assessments
- Determine individuals' needs for services and supports, eligibility, and level of care
- Evaluate individuals in homes, group homes, and care facilities to determine Long Term Care needs and placement options
- Perform duties in compliance with Person First standards, HIPAA guidelines, and confidentiality policies
- Ensure accurate and consistent documentation of assessment outcomes
- Travel to client, consumer, or program locations for in-person assessments exceeding 50% of the workweek
- Work in support of Maximus's Statewide Assessing Services program for Maine DHHS/OADS
Requirements
What you’ll need- Specific clinical license and experience requirements based on contract requirements
- Minimum two (2) years of post graduate nursing experience
- Ability to travel on-site to client, consumer, or program locations for more than 50% of the workweek
- Ability to commute within and cover Somerset County, Maine
- Reliable high-speed internet service: minimum 20 Mbps download / 50 Mbps for shared internet connectivity and minimum 5 Mbps upload
- Private and secure workspace
- Preferred: experience in long term care or home health care
- Preferred: experience with OASIS or MDS
