About the role
Key responsibilities & impact- Complete medical and/or behavioral health assessments within contract requirements
- Conduct on-site, video call, and/or telephonic assessments to determine individuals' needs for services and supports, eligibility, level of care, or related outcomes
- Conduct field-based IDD and SIS assessments
- Ensure quality and compliance standards are met
- Perform duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies and procedures
- Travel as required by program contract requirements, including 75% travel within Cumberland and York counties, Maine
- Support related program responsibilities
- Participate in regular monthly team meetings
- Perform other related duties as assigned
Requirements
What you’ll need- High School Degree or equivalent required
- Minimum 1 year of clinical experience required
- Bachelor's degree in Human Services or related field
- Minimum 1 year of direct experience working with individuals with intellectual and/or developmental disabilities or Autism
- Experience conducting assessments in the field
- Knowledge and understanding of medical and/or behavioral health diagnoses and prescribed medications
- Ability to collect data, define problems, establish facts, and draw valid conclusions
- Preferred knowledge of community support programs, long-term care assessment, and level of care in medical, behavioral health, or related programs
- Obtain SIS-A certification upon completion of training
- QIDP or similar experience helpful
- ISP experience a plus
- Reliable high-speed internet service
- Minimum 20 Mbps download speed, or 50 Mbps for shared internet connectivity
- Minimum 5 Mbps upload speed
- Private and secure workspace
- Ability to travel 75% within Cumberland and York counties, Maine
