About the role

Key responsibilities & impact
  • Complete medical and/or behavioral health assessments within contract requirements
  • Conduct on-site, video-call, and/or telephonic assessments to determine individuals' needs for services and supports, eligibility, level of care, or related outcomes
  • Manage the SIS assessment process from scheduling through facilitating assessment meetings, documenting findings, and maintaining communication with families and providers
  • Partner with individuals, families, providers, and community stakeholders
  • Travel throughout the assigned geographic region as required by the program contract
  • Perform duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies and procedures
  • Perform other related duties as assigned

Requirements

What you’ll need
  • High School Degree or equivalent required
  • Minimum 2 years of clinical experience required
  • Minimum 2 years of experience working with individuals with Intellectual Disabilities or Developmental Disabilities (ID/DD) with Mental Health Conditions
  • Must reside within Knox County or within a one-hour commuting radius of Knox County
  • Bachelor's degree in Psychology, Social Work, Human Service, Nursing, Special Education, or Mental Health
  • Understanding of Person-First Language when interacting with individuals with disabilities, families, caregivers, and community partners
  • Computer proficiency and ability to navigate multiple systems and applications efficiently
  • Proficiency in Microsoft Office applications
  • Ability to multitask and manage competing priorities effectively
  • Strong verbal and written communication skills
  • Excellent organizational and follow-up skills
  • Strong attention to detail and documentation accuracy
  • Valid driver's license and reliable transportation for field-based work
  • Reliable high-speed internet service
  • Minimum 20 Mbps download speed / 50 Mbps for shared internet connectivity
  • Minimum 5 Mbps upload speed
  • Private and secure workspace
  • Preferred: Minimum 2 years of experience working with individuals with Serious Mental Illness (SMI)
  • Preferred: Prior experience conducting clinical assessments
  • Preferred: Familiarity with medical terminology and diagnoses
