About the role

Key responsibilities & impact
  • Manage the intake and assessment preparation process
  • Conduct telephonic screenings

  • Facilitate timely scheduling of assessments
  • Serve as the primary point of contact for members after an assessment request is initiated
  • Support a smooth, person-centered member experience
  • Perform cross-trained Customer Service Representative functions and provide front-line member support as needed
  • Review and validate data and reports
  • Work assigned reports to identify necessary fixes or work-arounds
  • Make recommendations to improve processes
  • Ensure assignments are current and properly documented
  • Use tracking tools to record assignments, processes, and defects
  • Review, research, and audit system results

Requirements

What you’ll need
  • High School diploma, GED, or equivalent required
  • 2-4 years of relevant experience required
  • Minimum 3-5 years of experience in healthcare, human services, care coordination, customer service, case management, or related field
  • Residency in the state of Massachusetts
  • Proficiency in Microsoft Office applications
  • Proficiency in computer skills and ability to navigate multiple systems and applications efficiently
  • Ability to multitask and manage competing priorities effectively
  • Strong verbal and written communication skills
  • Excellent organizational and follow-up skills
  • Strong attention to detail and documentation accuracy
  • Valid driver's license and reliable transportation for minimal field travel
  • Reliable high-speed internet service
