About the role

Key responsibilities & impact

Complete medical and/or behavioral health assessments within contract requirements
Conduct on-site, video call, and/or telephonic assessments to determine individual needs, service and support eligibility, level of care, or related outcomes
Conduct virtual evaluations using the Time for Task (TTT) Tool
Observe and evaluate functional abilities, ADLs, IADLs, cognitive status, mobility, safety risks, and support needs
Document assessment findings and submit completed assessments using electronic assessment systems (e-CAS)
Ensure assessments meet organizational standards, regulatory requirements, and evidence-based clinical guidelines
Support clinical excellence and the Massachusetts EOHHS program mission
Travel and perform occasional field-based, in-home, or community-based assessments as required
Perform all duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies
Collaborate with the Public Assessors Option and Massachusetts EOHHS/MassHealth LTSS program operations
Perform other related duties as assigned

Requirements

What you’ll need

Master's degree required
Master's degree in Occupational Therapy
Current licensed therapist valid in the state of practice is required
Active, unencumbered Occupational Therapy (OT) license in Massachusetts
Minimum of 5 years of Occupational Therapy experience or equivalent relevant experience assessing functional status, mobility, cognition, and activities of daily living
Knowledge of community support programs, long-term care assessment, and level-of-care programs preferred
Knowledge of medical and/or behavioral health diagnoses and prescribed medications
Ability to collect data, define problems, establish facts, and draw valid conclusions
Excellent written and verbal communication skills
Excellent interpersonal and customer service skills
Proficiency in Microsoft Office Suite and applications
Excellent organizational skills and attention to detail
Ability to work in a fast-paced environment
Ability to work independently and meet timelines
Valid driver's license and reliable transportation for community-based assessments
Proficiency in Adobe applications preferred
Proficiency in Smartsheet preferred
Familiarity with the Time to Task (TTT) assessment preferred
Knowledge of functional and occupational performance assessments preferred
Experience working with elderly adults and individuals with disabilities preferred
Understanding of MA EOHHS LTSS program clinical eligibility, assessment documentation, caregiver and surrogate participation, and home and community-based services preferred
Reliable high-speed internet service
Minimum 20 Mbps download speed, or 50 Mbps for shared internet connectivity
Minimum 5 Mbps upload speed
