About the role

Key responsibilities & impact
  • Complete medical and/or behavioral health assessments within contract requirements
  • Conduct on-site, video call, and/or telephonic assessments
  • Determine individuals’ needs for services and supports, eligibility, level of care, or related outcomes
  • Evaluate individuals in homes, group homes, and care facilities to determine long-term care needs and placement options
  • Perform duties in compliance with Person First standards, HIPAA guidelines, and confidentiality policies
  • Travel as required by program contract requirements
  • Perform other related duties as assigned

Requirements

What you’ll need
  • Current Registered Nurse (RN) license valid in the state of practice required
  • High school degree or equivalent required
  • Minimum 1 year of clinical experience required
  • Minimum 2 years of postgraduate nursing experience
  • Ability to commute within and cover Cumberland County
  • Experience in long-term care or home health care preferred
  • Experience with OASIS or MDS preferred
  • Reliable high-speed internet service: minimum 20 Mbps download and 5 Mbps upload
  • Private and secure workspace
  • Ability to comply with Person First standards, HIPAA guidelines, and company confidentiality policies
  • Travel may be required based on program contract requirements
