About the role
Key responsibilities & impact- Complete medical and/or behavioral health assessments within contract requirements
- Conduct on-site, video call, and/or telephonic assessments
- Determine individuals' needs for services and supports, eligibility, level of care, or related outcomes
- Evaluate individuals in homes, group homes, and care facilities to determine Long Term Care needs and placement options
- Perform duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies
- Travel as required by program contract requirements
- Perform other related duties as assigned
Requirements
What you’ll need- Current Registered Nurse (RN) license valid in the state of practice is required
- High school degree or equivalent required
- Minimum 1 year of clinical experience required
- Minimum 2 years of post graduate nursing experience
- Ability to commute within and cover Waldo and Kennebec counties
- Ability to conduct on-site, video call, and telephonic assessments
- Compliance with Person First standards, HIPAA guidelines, and company confidentiality policies and procedures
- Reliable high-speed internet service: minimum 20 Mbps download/50 Mbps for shared internet connectivity and minimum 5 Mbps upload
- Private and secure workspace
- Experience in long-term care or home health care preferred
- Experience with OASIS or MDS preferred
