About the role

Key responsibilities & impact

Complete medical and/or behavioral health assessments within contract requirements
Conduct on-site, video call, and/or telephonic assessments
Determine individuals' needs for services and supports, eligibility, level of care, or related outcomes
Evaluate individuals in homes, group homes, and care facilities to determine Long Term Care needs and placement options
Perform duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies
Travel as required by program contract requirements
Perform other related duties as assigned

Requirements

What you’ll need

Current Registered Nurse (RN) license valid in the state of practice is required
High school degree or equivalent required
Minimum 1 year of clinical experience required
Minimum 2 years of post graduate nursing experience
Ability to commute within and cover Waldo and Kennebec counties
Ability to conduct on-site, video call, and telephonic assessments
Compliance with Person First standards, HIPAA guidelines, and company confidentiality policies and procedures
Reliable high-speed internet service: minimum 20 Mbps download/50 Mbps for shared internet connectivity and minimum 5 Mbps upload
Private and secure workspace
Experience in long-term care or home health care preferred
Experience with OASIS or MDS preferred
