About the role
Key responsibilities & impact- Ensure coordination with Henderson KY Ryan White Part B and Housing Program Manager and across HIV care and housing programs to deliver seamless services aligned with Ryan White Part B, HOPWA, and TBRA guidelines.
- Responsible for drafting and updating departmental polices that align with funding requirements.
- Oversee program implementation in accordance with state and federal funding requirements, ensuring accurate service documentation, eligibility tracking, and outcome alignment.
- Monitor the delivery and impact of housing services, including rental assistance, housing case management, and supportive services to promote long-term housing stability for people living with HIV.
- Maintain and strengthen partnerships with health departments, housing agencies, and service providers to support referrals, shared clients, and coordinated funding efforts.
- Analyze program data to track progress, identify trends, and support improvements in service delivery and client outcomes.
- Maintain accurate records of programmatic MOUs, contracts, and documentation required by funding agencies across both Part B and housing programs and will ensure the objectives listed within the funding workplan for Kentucky Part B are being met.
- Will participate in local planning and activity boards (for example- KHPAC, Homeless connection, etc.)
