About the role
Key responsibilities & impact- Perform social work assessments and interventions for inpatients, outpatients, and emergency department patients
- Provide psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues
- Formulate biopsychosocial assessments, disposition plans, and treatment plans
- Collaborate with healthcare professionals to evaluate medical or physical conditions and assess client needs
- Monitor, evaluate, and record client progress against measurable treatment and care plan goals
- Develop and implement patient-centered treatment plans addressing medical, social, and resource issues
- Work collaboratively with healthcare team members, patients, families, and interdisciplinary teams
- Advocate for clients while maintaining professional boundaries
- Coordinate and communicate treatment plans
- Facilitate skill-based groups
- Engage in quality improvement projects and use data to measure progress
- Attend department meetings approximately once per month at Assembly Row in Somerville
- Travel to see patients in the Boston community 3–5 days per week
Requirements
What you’ll need- Master's Degree in Social Work required
- Valid Driver’s License and reliable transportation required
- LCSW, LICSW, or LMHC licensure required
- 2+ years of clinical experience in a medical setting required
- Proficient clinical judgment and application of professional values and ethics
- Ability to formulate risk assessments, crisis interventions, psychosocial assessments, treatment plans, and timely documentation
- Sound clinical judgment and ability to advocate for clients while maintaining professional boundaries
- Working knowledge of internal and external resources
- Ability to provide evidence-based interventions
- Ability to advocate, coordinate, and communicate treatment plans with patients, families, and interdisciplinary teams
- Ability to use supervision and consultation appropriately
- Ability to work in a complex organization
- Ability to communicate findings with a broader audience
- Ability to use data to measure progress
- Must use a stable, secure, compliant workstation in a quiet environment on remote workdays
