About the role

Key responsibilities & impact

Assess individual needs regarding social determinants of health
Coordinate care and connect clients with appropriate services and resources
Help clients build coping skills, self-determination, and personal empowerment
Work collaboratively with a multidisciplinary care team

Requirements

What you’ll need

Bachelor's Degree in Business required or Bachelor's Degree in a related field of study required
Five years of experience required, of which two must be related to healthcare
Experience working with substance use disorder recovery services
Excellent problem-solving skills
Ability to work independently and as part of an interdisciplinary team
Ability to apply risk management and problem-solving techniques to clinical situations
Ability to thrive in a fast-paced, team-oriented environment
Comfort working with clients in crisis situations
Basic knowledge and understanding of the current version of DSM is a plus
Basic knowledge and understanding of psychiatric medications is a plus
Skillset or interest in working with corrections and women's health
