About the role

Key responsibilities & impact
  • Provide direct patient care within an ambulatory setting, practicing under approved Collaborative Practice Agreements (CPAs).
  • Focus on comprehensive medication management, chronic disease state management, medication access optimization, and population health initiatives.
  • Collaborate closely with primary care providers and the interdisciplinary care team to initiate, modify, or discontinue medication therapy in accordance with evidence-based protocols.
  • Contribute to quality assurance activities and continuous improvement efforts to advance ambulatory care services.

Requirements

What you’ll need
  • Current California Board of Pharmacy – Licensed Pharmacist required.
  • Doctor of Pharmacy (PharmD) required.
  • Post-Graduate Year 1 (PGY1) Pharmacy required.
  • Post-Graduate Year 2 (PGY2) Pharmacy Residency preferred.
  • Board Certification (BCPS, BCACP) preferred.
  • Basic Life Support (BLS) Certification required.
  • Experience working in an ambulatory care or primary care clinic setting providing direct patient care required.
  • Knowledge of ambulatory pharmacotherapy and clinical decision-making, including evaluation of laboratory data and medication use.
  • Experience identifying medication-related problems and recommending evidence-based interventions to optimize therapy.
  • Skill in reviewing patient medical records and developing individualized medication management plans.
  • Ability to communicate patient care plans, clinical recommendations, and medication-related concerns effectively to physicians and other healthcare professionals.
