About the role
Key responsibilities & impact- Provide care coordination from referral and test scheduling through treatment options for patients undergoing TAVR, LAAC, TEER, and PFO/ASD closures
- Provide patient education and coordinate care with physicians and surgeons across disciplines
- Complete documentation and compliance requirements
- Perform nursing assessment, identify problems, plan care, report complex clinical data to cardiologists, and implement care plans as directed
- Manage transitions of care from the acute setting to home
- Support Care Team duties as assigned
- Maintain patient follow-up according to CMS and NCDR guidelines
- Maintain patient referral and database using Microsoft Excel and Teams
- Maintain insurance and NCDR compliance documentation
- Educate patients about cardiovascular conditions, treatment options, prevention, self-monitoring, and resources
- Triage physical symptoms and arrhythmias and consult cardiologists per protocol
- Participate in Transition of Care Management and update medications, problem lists, and follow-up in the Electronic Health Record
- Review physician tasks, clinical worklists, preoperative lab results, and imaging results
- Assist the Clinical Program Director and Structural Heart Team with nursing practices, policies, and care protocols
- Maintain clinic safety equipment and emergency cart
- Ensure documentation is audit-ready
- Collaborate with clinical managers, physicians, and staff to promote patient safety and protocol adherence
- Identify solutions to work-related problems and improve services
- Cover other staff as necessary
- Participate in clinic meetings, special projects, educational seminars, and community activities
- Respond to emergencies and support the team in acute situations
Requirements
What you’ll need- Bachelor's Degree of Science in Nursing from an accredited college/university
- Minimum of two-year cardiovascular clinic experience and/or hospital nursing experience
- Two years critical care hospital experience preferred
- Two years case management experience in the hospital or clinic setting preferred
- Excellent written and oral modes of interpersonal communication
- Maintains CPR and ACLS certification
- Current Registered Nurse California
- Measles, mumps, varicella, and annual influenza immunizations are conditions of employment
- 40 scheduled weekly hours
- 8-hour day shift
