About the role
Key responsibilities & impact- Perform patient chart reviews to identify opportunities to satisfy HEDIS measure gaps
- Maintain systems for storing and tracking HEDIS medical record documentation
- Document in the Electronic Medical Record
- Request charts and medical records from other facilities to close care gaps
- Scan or upload medical records into payor databases
- Conduct patient outreach to schedule Annual Wellness Visits and follow-up appointments
- Perform pre-visit and post-visit evaluations to identify gaps in care
- Educate clinic staff about metrics and closing HEDIS gaps
- Identify, track, and follow up on unresolved care gaps
- Perform medication adherence reviews and mitigate barriers
- Assist with processing hierarchical condition category attestation forms from payors
- Implement continuous quality improvement projects and workflow improvements using DMAIC and other quality tools
- Create and publish team workflows
- Organize and maintain internal files and folders
- Provide in-service training and precepting to Associate Level QI specialists
- Create data visualizations for HEDIS metrics and medication adherence
- Report team data at leadership meetings
- Meet with payor representatives regarding gap-closure strategies
- Lead quality improvement projects and root cause analyses related to quality metrics
- Perform other duties as assigned
Requirements
What you’ll need- Support the mission, vision, and patient promise throughout all interactions
- Communicate effectively, both written and verbally
- Demonstrate analytical thinking and use logic to coordinate work within schedule constraints and handle emergent requirements
- Participate as an integral part of the interdisciplinary healthcare team
- Working knowledge of HEDIS measures for which UT Health is responsible under shared savings programs
- High school diploma or GED
- Three years of experience as a Quality Improvement Specialist or Medical Assistant
