About the role

  • Oversee the full lifecycle of care coordination for members in the Virtual Intensive Outpatient Program (VIOP)
  • Screening and qualifying prospective members for program eligibility
  • Conduct clinical outreach to families and onboarding new members
  • Manage case coordination and facilitate communication across multidisciplinary teams
  • Support members and caregivers throughout their VIOP journey
  • Ensure timely scheduling of intake assessments once members are qualified for VIOP enrollment
  • Orient members and their families to the VIOP program
  • Serve as the primary point of contact for members and families
  • Coordinate external case management needs with schools, Primary Care Providers, and community resources
  • Assist with discharge planning and transition to lower levels of care or additional services
  • Maintain accurate and timely documentation in the electronic health record (EHR)
  • Act as a liaison between families and the VIOP treatment team
  • Collaborate with internal teams to address barriers to care
  • Conduct comprehensive biopsychosocial assessments for members entering the VIOP
  • Develop individualized care plans in collaboration with the treatment team
  • Support group facilitators during sessions
  • Provide clinical session coverage as needed

Requirements

  • Master’s degree in Social Work (MSW) and Associate Registration (or state equivalent)
  • Experience working in intensive outpatient programs or high acuity behavioral health settings within a multidisciplinary team preferred
  • Experience working with pediatric and adolescent populations, including families
  • Computer and live video tools literacy (e.g., Zoom, Google Meets, DoxyMe, Skype, etc.)
  • Skilled in case management, care coordination, and collaborative care delivery
  • Organized and excited by a dynamic environment
  • Passionate about helping kids and teens achieve goals
  • Ability to work full-time, approximately 40 hours per week
  • Candidates must successfully pass a drug screen and background check prior to employment for this role
  • Full-time resident of the United States
  • Additional language skills are beneficial to this role
